Question about your bill 

What billing or insurance information will I receive?

You will receive an explanation of benefits (EOB) from your insurance company. This may be a preliminary bill with only charges or you may get a final document which will display both charges billed and payment. 

How long will it take to get procedures approved with the insurance company?

There is no set time period for this. It varies depending on your insurance benefits, place of service, and the procedure itself. We are happy to update you on the progress at any time.

My doctor performed a procedure outside the office. Why am I getting so many different bills?

Each provider (doctor or nurse practitioner) who treated you during your procedure will bill your insurance. Depending on your insurance benefits, your coverage may differ from provider to provider. The hospital or surgery center where your procedure was performed will also bill your insurance.

I received a notice that my insurance company has paid on my bill - but I can't understand how they calculated their payment amount. Do you know?

The amount your insurance company pays is dependent on either a negotiated amount if out of network or as per the contract that the provider or hospital/ambulatory surgery center has with your insurance plan.

Can the estimate for my procedure differ from the actual billed amount and why?

The estimate you received prior to your procedure was based on anticipated charges. Actual charges can differ based on what was done during your procedure. We always will give you the closest estimate.

At Liberty Pain Associates, we are IN network with most popular plans. This means that we have a contract with the insurance company and also lower costs to our patients. We accept out of network insurance plans but the reimbursement rates are negotiated.
